Job Description : \n\nRN Case manager needed for Fort Worth office.\n\nMonday-Friday, hospice experience is required.\n\nIdentify the patient's and family's physical, psychosocial, environmental, safety and developmental needs and individualizing the patient's plan of care based on the needs identified.\n\n \n\n Required Qualifications :

\n1-year recent (within the last 3 years) full-time equivalent experience

\nGraduate of an accredited nursing program.

\nLicense, Certification, Registration

\nRegistered Nurse License (in the state where care is provided)

\nBLS (AHA)
