Position Summary
The IHS staff provides person-centered support to adults with disabilities living in their own homes or family homes. Services focus on assistance, supervision, and (if authorized) skill-building to promote safety, independence, and community inclusion. Staff follow the Coordinated Service and Support Plan (CSSP), CSSP Addendum, and agency procedures in accordance with MN 245D standards.
Note : This description can be used for IHS—Without Training or IHS—With Training . Include or remove the “Teaching / Skill Training” duties based on the service authorization.
Essential Duties & Responsibilities
Person-Centered Support
- Assist with ADLs / IADLs (meal prep, household tasks, hygiene prompts, medication reminders per policy, budgeting, shopping, scheduling, transportation to activities / appointments).
- Follow risk management and positive support strategies; implement any formal plans (e.g., Positive Support Transition Plan, Behavioral Support Plan).
Teaching / Skill Training (IHS—With Training only)
Implement teaching strategies and collect data for goals in the CSSP / CSSP Addendum (e.g., safety skills, community navigation, self-advocacy, communication, coping skills).Document progress per plan expectations.Health & Safety
Maintain a safe environment; follow universal precautions and infection control.Recognize and report abuse / neglect / exploitation as a mandated reporter .Follow emergency procedures; complete incident reports on time.Documentation & EVV
Use the agency Electronic Visit Verification (EVV) to clock in / out with accurate location, start / stop times, and service notes.Write 245D-compliant notes (date, start / stop, service type, interventions, outcomes / response, signature).Protect confidentiality (HIPAA) and maintain professional boundaries.Communication & Teaming
Communicate promptly with supervisor about health / behavior changes, schedule issues, or incidents.Participate in check-ins, required trainings, and team meetings.Transportation (if authorized)
Provide safe transportation using an insured vehicle; maintain a satisfactory driving record and mileage logs per policy.Minimum Qualifications
18+ and eligible to work in the U.S.; High school diploma / GED.Successful MN background study (NETStudy 2.0) and MVR check (if transporting).Ability to read / write clear documentation in English; proficient with smartphones / apps, EVV, email, secure messaging, uploading documents / photos, and basic troubleshooting.Reliable transportation, valid driver’s license, and auto insurance (if transporting).Physical ability to perform required tasks (standing, bending, light lifting up to [__] lbs.).Must be able to perform all essential functions of the role without restriction , including required physical and documentation tasks.
