This facility operates a comprehensive PACE (Program of All-Inclusive Care for the Elderly) program serving seniors who need nursing home-level care but can safely remain in the community. The program provides the entire continuum of care and services to seniors with chronic needs while maintaining their independence at home for as long as possible.

Job Details :
Full-time Medical Director position for community-based PACE program Conduct history and physical exams on new participants with event-based and periodic reassessments Develop medical treatment plans and provide medically necessary services across various locations Interview and evaluate applicants to PACE organization and participate in Intake and Assessment decisions Discuss medical conditions and prognosis with participants and caregivers to assist with care planning Collaborate in nursing treatment programs and participate as an active member of the interdisciplinary team Educate participants and families about health maintenance and chronic medical conditions Supervise healthcare and treatment programs developed by the interdisciplinary team Perform physical examinations, order appropriate tests, and document results systematically Participate in provider on-call rotation as required Must have current and active Florida medical license Board Certification in Internal Medicine or Family Practice required (Geriatrics certification preferred) Experience in geriatrics preferred with minimum one year documented experience with frail / elderly population Current DEA Registration and CDS Certification required Previous supervisory experience required Must possess effective skills in physical assessment and chronic disease management for frail older adults
Compensation :
