Position Summary :

We are seeking an experienced and detail-oriented MDS Coordinator to oversee and manage the completion and submission of accurate Minimum Data Set (MDS) assessments. This role is essential in ensuring proper reimbursement and compliance with federal and state guidelines.

Responsibilities :

Complete and submit MDS assessments in a timely and accurate manner

Ensure compliance with all regulatory requirements and facility policies

Coordinate interdisciplinary team (IDT) meetings and care planning processes

Monitor changes in resident conditions and adjust assessments accordingly

Provide education and support to nursing staff on documentation and coding best practices

Qualifications :

Current NY State RN license required

Minimum 2 years of experience in MDS coordination or a similar role in long-term care

Knowledge of Medicare and Medicaid regulations related to MDS

Strong analytical and communication skills

Proficiency with MDS software and electronic health records (EHR)
