Hospice : RN Visit Nurse (West Metro)

Description

The Hospice RN Visit Nurse is responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education / experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.

Position : RN Visit Nurse for Hospice

Salary Wage Range : $85,000-$91,300 annually, depending on experience

Service Area : West Metro (coverage area : Waconia, Carver, Chaska, Jordan, Chanhassen, Savage, Minnetonka, Buffalo, Champlin, Loretto, Anoka, Osseo, Rogers, Elk River, Rogers, Ramsey, Maple Grove, Zimmerman, Albertville, Bethel, North Loop / North Minneapolis, Fridley, Columbia Heights, Coon Rapids, Blaine, Brooklyn Center, Ham Lake, Andover, East Bethel, Golden Valley, Robbinsdale, Brooklyn Center, Brooklyn Park, Anoka, Coon Rapids, Oak Grove & surrounding). Plus mileage reimbursement of .70 cents per mile.

Schedule : 1.0 FTE, Monday - Friday 8 : 00am-5 : 00pm, 2 holiday rotations / year & no weekends, no on-call
