Home Health Physical Therapist (PT)

About the Role :

Medadventures is seeking an experienced and compassionate Physical Therapist to provide one-on-one care in a home health setting. This role is ideal for a clinician who values flexibility, autonomy, and meaningful patient relationships.

Responsibilities :

Evaluate, plan, and implement individualized treatment programs for patients in their homes.

Collaborate with the interdisciplinary care team to ensure continuity of care.

Document all visits and progress notes in compliance with home health regulations.

Educate patients and caregivers on safe mobility, exercise, and self-care techniques.

Qualifications :

Active state Physical Therapy license.

Minimum of one year of clinical experience (home health experience preferred).

Strong communication and time-management skills.

Reliable transportation and valid driver’s license.
