Location : Walnut Creek, CA

Job Description :

Join our team as a Home Health Physical Therapist, focusing on geriatric care. This 13

  • week contract, with potential extension, runs Monday to Friday, 8 : 00 am to 5 : 30 pm.

    • Responsibilities :

  • Provide quality physical therapy to home health patients.
  • Manage a caseload of about 6 patients daily.
  • Schedule and coordinate patient visits.
  • Collaborate with RNs and OT / PT staff.
  • Use EMR systems like Oasis and HCHB (training available).

    • Requirements :

  • Valid CA PT license.
  • BLS certification.
  • At least 1 year of home health experience.
  • Preferably experienced with geriatric patients.

    • Additional Information :

  • No on
  • call or holiday duties.
  • Flexible scrub color.
  • Includes HR, HCHB training, and mentorship.
