California Licensed Nurse Practitioner

A company is looking for a Nurse Practitioner - Part-Time - Remote (CA).

Key Responsibilities

Develop and implement individualized care plans in collaboration with patients and interdisciplinary teams

Provide clinical care for chronic and acute conditions, including telehealth and in-home visits

Address behavioral health concerns and social determinants of health by connecting patients with community resources

Required Qualifications, Training, and Education

Active and unrestricted licensure as a Nurse Practitioner (NP), Physician Assistant (PA), or Medical Doctor (MD / DO) in California

Minimum of 3 years of clinical experience with a focus on complex care or population health

Experience in primary care, preferably with geriatric and palliative care patients

Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatrics preferred

Familiarity with telemedicine platforms and digital health tools
