Through collaboration between our Nurse Practitioners, Physicians, Therapists, and Social Workers, we are building a team-based approach to whole-person care.

Position Overview :

We are currently seeking Licensed Social Workers who are passionate about working with underserved populations and ready to make a direct impact in the lives of adults and older adults. This is a contract-based, per diem role to start—ideal for professionals seeking flexibility, autonomy, and purpose-driven work.

You’ll meet virtually with clients to assess mental health needs, provide supportive counseling, and connect individuals with social and community resources that support their total well-being.

Key Responsibilities :

  • Build trusting, therapeutic relationships with patients in a virtual setting.
  • Conduct psychosocial assessments and develop individualized care plans.
  • Provide short-term counseling, goal planning, and referrals as needed.
  • Help patients navigate community-based resources (e.g., housing, food, legal aid, transportation, behavioral health).
  • Maintain accurate, confidential documentation in the EMR.
  • Collaborate with our care team (NPs, therapists, pharmacists, dietitians) to ensure continuity of care.
  • Participate in case conferences and contribute to a supportive clinical culture.
  • Maintain compliance with HIPAA and state regulations.

Qualifications :

  • Master’s degree in Social Work (MSW) or related field.
  • Licensed in Maryland, D.C., or Virginia as LCSW-C, LMSW, LICSW , or equivalent.
  • Experience with underserved, uninsured, or geriatric populations preferred.
  • Minimum of 2 years of clinical experience (telehealth experience a plus).
  • Strong interpersonal, organizational, and documentation skills.
  • Bilingual (especially Spanish-speaking) a plus.
