Developmental Pathways (DP) has an opportunity for a Case Manager - Active in our Case Management Department. The Case Manager - Active will manage a caseload of individuals receiving Waiver and State General Fund services and maintain documentation needed quarterly and annually. The Case Manager - Active will complete assessments to determine eligibility for services / supports and identify individual strengths, needs, concerns, and preferences through interviewing, observing, and utilization of standardized tools. The Case Manager - Active will complete the required documentation related to monitoring and monitoring activities to advocate for individual service needs and ensure the effectiveness of services delivered (as authorized by the service plan). The Case Manager - Active will facilitate interdisciplinary teams (IDT) meetings to coordinate services to meet individual needs; ensure appropriate interaction and communication; and as appropriate to modify services. Lastly, will provide case management services and support to temporarily assigned individuals; and support with department and team coverage as needed.
This is a 40-hour per week, hybrid position, with the benefit of working remotely and in-person coming into our headquarters office in Aurora, CO and / or off-site locations

Requirements
Five (5) years of experience in the field of Long-Term Services and Supports; or
Upon request, qualifying need, and approval, driving requirements may be waived in lieu of reliable transportation.
