Now Hiring a Social Worker / Discharge Planner at Casselman Healthcare & Rehabilitation!

Duties and Responsibilities :

  • Completing resident assessments, coordination with service providers, and ensuring regulatory compliance
  • Identifying individual social and emotional needs
  • Developing and maintaining individualized care plans
  • Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
  • Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
  • Complete new admission paperwork and coordinate with   the business office to ensure resident’s financial and insurance needs are in place for the resident during their stay
  • Work with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the community

    • Qualifications / Requirements :

  • BSW   preferred, or bachelor's degree in related field
  • At least one year experience in Social Work field required
  • Experience in long term care and / or sub-acute settings preferred
