Duties and Responsibilities :
Completing resident assessments, coordination with service providers, and ensuring regulatory complianceIdentifying individual social and emotional needsDeveloping and maintaining individualized care plansParticipating in the planning of the resident’s admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentComplete new admission paperwork and coordinate with the business office to ensure resident’s financial and insurance needs are in place for the resident during their stayWork with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communityQualifications / Requirements :
BSW preferred, or bachelor's degree in related fieldAt least one year experience in Social Work field requiredExperience in long term care and / or sub-acute settings preferredJoin us as the Director of Social Services today!
