Schedule : Full-time (negotiable)

Patient Volume : 1520 patients / day (non-negotiable)

On-Call : 10 days / month

Patient Population : 100% adult

Practice Breakdown : 95% outpatient

5% inpatient

0% rehab / 0% sleep (sleep optional if desired)

Trauma cases : 05%

Support Staff : 1 EMG Tech

1 LPN

Shared receptionist

Group Size : 50 physicians, 10 APPs

Procedures / Equipment :

EMG onsite

Lab & X-ray onsite

EMR : Epic

Residency Requirement : Must live within 40 miles or be willing to relocate

Visa Candidates : H-1B and J-1 accepted Compensation & Benefits

Base Salary : Competitive (discussed at interview)

Sign-On Bonus : Negotiable

Contract Structure :

Years 12 : Guaranteed base + bonus incentive

Year 3 : Productivity model + bonus incentive

CME : $5,000 annual allowance
