The main function of the Resident Care Coordinator is to coordinate, lead, and direct the care and services provided to residents by nursing assistants, med techs and other professionals or paraprofessionals. May also provide direct resident care as required.

Your Responsibilities
Ensure all residents are treated with respect and dignity, recognizing individual needs and encouraging independenceAssist in the management of the healthcare of all residents within the community, including the dissemination of information to families and staffAssist in medication management and care planning process as assignedAssist the Director of Nursing or Executive Director in overseeing nursing staff - hiring, disciplines, scheduling, etcAssist with facility procedures for admission, discharge and transfer of residentsConsistently work cooperatively with residents, residents' representatives, facility staff, physicians, consultant personnel and ancillary service providersReports any issues or problems that may arise to the Director of Nursing or Executive DirectorAssists families and other visitors as neededComplies with state, federal, and all other applicable health care and safety standardsPromotes and exemplifies the Priority Life Care mission and values at all timesQualifications
