Your Role as a Home Health Occupational Therapist
In this rewarding position, you will :
Assess and screen patients' daily living and work-related skills, developing therapeutic retraining programs with measurable objectives.
Administer and interpret diagnostic tests to assist physicians in evaluating patients' functional levels.
Collaborate with physicians and healthcare team members to develop and revise the Plan of Care Treatment.
Provide therapeutic treatment and instruction to improve strength, coordination, range of motion, and overall function.
Review and expand retraining programs in line with each patient's behavioral goals.
Consult with vocational team members to create and implement vocationally oriented plans tailored to patients' needs.
Report patient status and progress to physicians, rehabilitation staff, and family members.
Participate in care coordination and discharge planning, acting as a resource for other healthcare personnel.
Design community reintegration activities to assist clients in their physical and psychological adjustment.
Recommend and design adaptive equipment to enhance clients' living or working environments.
Train staff and family members to implement retraining programs, including daily living skills.
Supervise and evaluate Occupational Therapy Assistants and Home Health Aides in delivering occupational therapy services.
Document patient care observations, interventions, and evaluations accurately and thoroughly.
Use Your Skills to Make an Impact
Required Experience / Skills :
Degree from an accredited Occupational Therapy Program.
A minimum of six months of occupational therapy experience preferred.
Home Health experience is a plus.
Current and unrestricted OT licensure.
Current CPR certification.
Strong organizational and communication skills.
A valid driver’s license, auto insurance, and reliable transportation are required.
