About The Position

Job Title : Registered Nurse

Experience : Minimum 1 full year of adult home health experience or 1 full year of hospice experience required.

License : Must have a valid nursing license.

Certifications : CPR, BLS

Must-Have :

Must have recent specialty experience.

Must be proficient in OASIS and HomeCare HomeBase EMR.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Must be willing to travel up to 75 miles from the assigned care center.

Description :

Provides and directs the provision of home nursing care according to physician's orders.

Care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to the Clinical Manager.

Direct supervision and evaluation of Home Health Aides and Licensed Practical Nurses or Licensed Vocational Nurses is required.

Home visits are necessary.

Periodic office meetings will occur.

Average number of patients per day is 5-10.

Each type of visit and activity holds a different point value for productivity expectations.

Documentation and travel activities also contribute to these points.

No guaranteed hours are available; the contractor must hit weekly productivity points.

Types of cases include start of care, wound care, and follow-up visits.

All documentation must be completed within 24 hours of the appointment with the patient.

Delayed and incomplete documentation can impact billing and follow-up patient care.

Contractors are expected to accept every patient on their schedule regardless of the distance from previous appointments.

Local candidates are accepted at the same rate.
