A company is looking for a Telephonic Licensed Practical Nurse (LPN) (Call Center).
Key Responsibilities
Deliver ongoing telephonic support and monthly check-ins with assigned patients to review health data and promote care adherence
Analyze patient readings and document trends to identify potential health concerns requiring follow-up or escalation
Collaborate with physicians and clinical staff to coordinate care and ensure timely intervention when necessary
Qualifications and Skills
Active Licensed Practical Nurse (LPN) license and Compact Nursing License (NLC) are required
Prior experience in a clinical or call center environment focused on patient communication and documentation
Comfortable handling a high daily call volume while maintaining empathy and professionalism
Self-motivated and detail-oriented, thriving in a fast-paced, remote setting
Proficiency with EMR systems and digital patient monitoring tools is preferred
