Your Responsibilities

As a Home Health Occupational Therapist Assistant , you will :

• Provide therapy services under the supervision of a qualified Occupational Therapist, following the patient’s Plan of Treatment.
• Assist in implementing vocational and educational programs designed to enhance task performance and promote health.
• Design and adapt equipment and environments to improve function and independence.
• Deliver therapeutic treatment and instruction to patients as directed by the Occupational Therapist.
• Document observations and prepare clinical notes, maintaining accurate patient records.
• Instruct patients and families on home programs and the use of adaptive equipment.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities to support clients in their physical and psychological adjustment.
