Nurse Case Manager

2 years of clinical experience with an adult / geriatric population with excellent service excellence skills. Position is in the intake department and is responsible for processing all patient referrals, physician orders, triaging patient / family calls as well as calls from referrals sources including hospitals, MD offices, SNF's, etc, and providing support as needed to clinical team. Excellent collaboration skills necessary and the ability to demonstrate a professional and positive attitude during the care transition process.
