A company is looking for a Care Navigator (Virtual Healthcare) to support individuals with dementia and their families through telehealth services.

Key Responsibilities

Provide guidance, assistance, and resources to individuals and families navigating dementia care

Conduct comprehensive assessments to understand the needs and challenges of patients and caregivers

Collaborate with interdisciplinary teams to develop and update personalized care plans

Required Qualifications

Associate's or Bachelor's degree required, or relevant professional experience in lieu of a formal degree

Minimum of 2 years of professional experience working with the geriatric population

Prior experience with dementia or cognitive impairments preferred

Ability to work in a virtual environment with a private and quiet workspace
