About The Position

Experience : Minimum of 2 years in case management in a facility with a similar capacity

License : Valid state license required

Certifications : ACM or CCM and BSN preferred

Must-Have :

Experience with EPIC and Allscripts preferred.

Experience with discharge planning, care coordination, and needs assessment.

High acuity discharge experience required.

Description :

This is a full-time 40-hour position, Monday through Friday.

Case managers work either 7 : 00 AM to 3 : 30 PM or 8 : 00 AM to 4 : 30 PM based on unit needs.

Case managers cover all units except for certain specialty areas.

Tasks of case managers include completing assessments on all admitted patients.

Participation in interdisciplinary rounds is expected.

The role includes initiating referrals for home health, durable medical equipment, skilled nursing facilities, and long-term acute care hospitals.

On average, the daily census exceeds 1000 hospital beds.

Weekend work is required every other weekend.

Candidates must have experience with hospitals of similar capacity, specifically those with more than 1000 beds.

Academic experience is preferred for this role.

No first-time travelers will be accepted for this position.

Travel experience is required.

Candidates must float within the service line.

References are preferred at the time of application.

All travelers are expected to complete required compliance documentation promptly.
