LTSS Service Care Coordinator

Job Title : LTSS Service Care Coordinator

Duration : 3 months Contract (Contract to Hire)

Location : Remote - Source from San Antonio, TX

Shift Type : 8am-5pm CST Optional OT is available, not mandatory

Pay range : $18 - $20 per hour

Job Duties :

• Assists with developing ongoing long-term care plans / service plans and works to identify providers, specialist, and / or community resources needed for long-term care
• Coordinates as appropriate between the member and / or family / caregivers and the care provider team to ensure identified services are accessible to members
• Provides resource support to members and their families / caregivers for various needs (e.g. employment, housing, participant direction, independent living, justice, foster care) based on service assessment and plans
• Monitors care plans / service plans, member status and outcomes, as appropriate, and provides recommendations to care plan / service plan based on identified member needs
• Provides and / or facilitates education to long-term care members and their families / caregivers on procedures, healthcare provider instructions, service options, referrals, and healthcare benefits
• Full time may need to work sometimes past 5pm as older members or parents are home after 5pm but can flex during day for this.
• Remote position, but we do meet in office every 2-3 months for meetings and team engagement activities. New hires may need to go to office for initial training.

Years of experience required :

• Excellent customer service skills, strong computer skills, attention to detail, telephonic work, reliable internet connection
• Computer skills – high level of exp, important (Microsoft Office)
• Communication skills – high level of exp, will be on telephonic majority of time
• Decision-making skills – be able gather information via probing to make decisions

Disqualifiers :

• No experience working with Medicaid and special health care needs

Education / Certification Required :

• HS Diploma
