Home Health Physical Therapist - Springfield, IL

We're seeking a compassionate PT for in-home therapy services in Springfield. Enjoy flexibility while providing one-on-one therapy and mobility education to patients recovering or managing chronic conditions.

Primary Functions :

Conduct PT assessments and adjust plans as needed
Support caregivers with home safety education
Supervise PTAs and document all interventions

Work Schedule :

Monday–Friday with one weekend every six weeks

Holiday shifts on rotation

Travel : Must be willing to travel locally in Springfield and nearby towns.
