job_description.job_card.job_descriptionMedication administration and education Catheter care, ostomy care, trach care, and tube feedings Blood glucose monitoring and diabetic teaching Vital signs monitoring and general health assessment Implement the nursing care plan established by the RN Case Manager and physician. Observe, record, and report changes in the patient’s condition to the supervising RN and physician promptly. Ensure accurate and timely documentation in the electronic health record (EHR) per agency policy and Medicare standards. Provide patient and family education regarding disease management, medication adherence, and safety at home. Follow infection control and safety protocols at all times. Participate in case conferences, staff meetings, and in-service education as required. Maintain confidentiality and professionalism in all interactions with patients, families, and healthcare providers.
