RiseWell Community Services is seeking a compassionate Full-Time Social Care Navigator to join our team in the Social Care Network Department. In this role, the Social Care Navigator will be responsible for conducting HRSN screenings, eligibility assessments, and navigation activities for Medicaid Fee-for-Service and non-eligible Managed Medicaid members.

This is a grant funded position - location is remote however, applicant must be able to report to West Babylon, NY for trainings and or meetings.

The Social Care Network Department provides services that are designed to empower both adults and children / adolescents on their journey to recovery, RiseWell offers comprehensive outpatient mental health and addiction services to individuals in need. Our mission is to provide unwavering support and guidance to individuals seeking assistance through individual and group therapy, medication-assisted treatment, and comprehensive support services.

QUALIFICATIONS : Bachelor’s Degree preferred. A minimum of two years’ experience completing administrative tasks. Valid and clean NY State driver’s license. Fingerprinting, criminal record check, and approval from NYS Office of Mental Health.

SALARY : $47,500
