Advanced Practice Clinician

Location : Spokane, WA (Travel between multiple senior living facilities)

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM

Travel Required : Yes – 100% travel between senior living facilities during shift

Position Summary

Seeking an Advanced Practice Clinician (APC) to provide integrated, longitudinal care to high-risk health plan members in senior living facilities. This role offers flexible scheduling with no on-call, weekend, or holiday requirements, and is part of an interdisciplinary team focused on improving outcomes for patients with complex medical and behavioral needs.

Primary Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute and chronic medical and behavioral conditions
  • Ensure accurate documentation, including ICD-10 coding
  • Collaborate with interdisciplinary care teams
  • Lead advanced illness and care planning conversations
  • Provide education and counseling to patients and caregivers

Required Qualifications

  • Active and unrestricted NP or PA license in the state of practice (or ability to obtain by start date)
  • NP : Graduate of an accredited MSN or DNP program
  • PA : Graduate of an accredited PA program
  • Board certification :
  • NP : Certified by AANP or ANCC in Family, Adult, Gerontology, or Adult-Gerontology Acute Care
  • PA : Certified by NCCPA
  • Current DEA license and prescriptive authority
  • Ability to meet physical requirements (e.g., lifting 30 lbs., navigating stairs, kneeling for assessments)
  • Reliable transportation
  • Ability to obtain collaborative practice agreement if required by state
  • Full COVID-19 vaccination (per federal, state, and client requirements)
  • Drug screening required prior to employment
  • Preferred Qualifications

  • 1+ year of clinical experience (long-term care preferred)
  • Experience with complex behavioral, social, or functional patient needs
  • Understanding of geriatrics, chronic illness, and end-of-life care
  • Proficiency in electronic medical records and documentation
