Healthcare Setting : Home Health Start Date : November 10, 2025 Contract Duration : 13 weeks RN Shifts : Friday, Saturday, Sunday (Blocked Schedule) Shift Hours : 12-hour shifts, 3 shifts per week (36 hours / week) Estimated Weekly Pay : $2,003 - $2,226 Registered Nurse Qualifications Minimum 12 months of recent Home Health RN experience (within the last 3 years) Proficiency with HCHB (Homecare Homebase) – required OASIS documentation experience – required Active Wisconsin or Compact Registered Nurse License – required (must be in hand at time of submission) Current BLS certification – required Home Health RN Responsibilities Deliver skilled nursing care to patients in their homes across the Greater Milwaukee area Complete three patient admissions per day and approximately two additional home visits daily Utilize HCHB and OASIS for accurate, timely clinical documentation Collaborate with interdisciplinary healthcare teams to develop and implement individualized care plans Educate patients and families on disease management, medications, and self-care techniques Ensure compliance with all regulatory, clinical, and safety standards Discover the rewards of home health nursing jobs in Milwaukee, WI.
