Pay rate $35.00 per hour. Job Description Our Primary Care team is looking for an exceptional nurse to help us manage and coordinate care for our high-acuity patients.

Nurses are responsible for carrying out a variety of healthcare services, including :
- Medication management;
- Coordinating care between outside providers and hospital systems;
- Disease management;
- Wound care;
- Recording vitals;

- Phlebotomy;
- Hydration Therapy and more!
