Associate Management Analyst - Bridges To Wellness - Limited Term
San Mateo County Health's Public Health, Policy and Planning Division is seeking a well-qualified individual for the position of Associate Management Analyst - Bridges To Wellness - Limited Term to provide a wide variety of administrative, analytical, and work-flow support to the Bridges To Wellness Program. The Bridges To Wellness Program provides field-based, intermediate level care management and supportive services to individuals with complex needs including substance use disorders, mental health challenges, chronic medical conditions, justice involved, and unhoused.
Responsibilities include :
- Pull, clean, validate, and analyze data from multiple sources.
- Generate state-mandated reports on Enhanced Care Management services.
- Develop and maintain dashboards tracking client health outcomes, chronic disease measures, and emergency service utilization.
- Produce reports aligned with HEDIS measures where feasible.
- Track health issues such as heart disease, hypertension, diabetes, inpatient admissions / readmissions, and ED utilization.
- Monitor trends and outcomes to evaluate program effectiveness and identify improvement opportunities.
- Support leadership with data-driven insights for decision-making and compliance.
- Support the program's transition from Avatar to Epic for documentation and reporting.
- Help design and implement workflows consistent with ECM requirements.
- Ensure documentation standards are met to support accurate reporting and coordination of care.
- Work with ECM providers, IT, and program staff to ensure accurate data capture and reporting processes.
- Serve as a liaison between clinical / program teams and data / analytics teams.
- Ensure data integrity and compliance with state and federal reporting requirements.
- Establish repeatable processes for ongoing data quality monitoring and reporting.

Experience with reports required by state or federal governmental agencies.Experience conducting data analysis.Ability to conduct research and apply process improvement activities.Ability to work under pressure with short deadlines.Ability to work across divisions to provide support and coordinate work.Excellent technical writing, editing, and proofreading skills.Demonstrated attention to detail in processes and execution of work.Proficient in computer systems and programs; specifically advanced level Excel, Power BI and other Microsoft Office Suite and Applications.Experience with using statistical software e.g. R, SAS, Stata and Visio or similar platforms for data analysis and workflow creation.Extensive verbal and written communication skills.This is a limited term, at-will position.
