Associate Management Analyst - Bridges To Wellness - Limited Term

San Mateo County Health's Public Health, Policy and Planning Division is seeking a well-qualified individual for the position of Associate Management Analyst - Bridges To Wellness - Limited Term to provide a wide variety of administrative, analytical, and work-flow support to the Bridges To Wellness Program. The Bridges To Wellness Program provides field-based, intermediate level care management and supportive services to individuals with complex needs including substance use disorders, mental health challenges, chronic medical conditions, justice involved, and unhoused.

Responsibilities include :

  • Pull, clean, validate, and analyze data from multiple sources.
  • Generate state-mandated reports on Enhanced Care Management services.
  • Develop and maintain dashboards tracking client health outcomes, chronic disease measures, and emergency service utilization.
  • Produce reports aligned with HEDIS measures where feasible.
  • Track health issues such as heart disease, hypertension, diabetes, inpatient admissions / readmissions, and ED utilization.
  • Monitor trends and outcomes to evaluate program effectiveness and identify improvement opportunities.
  • Support leadership with data-driven insights for decision-making and compliance.
  • Support the program's transition from Avatar to Epic for documentation and reporting.
  • Help design and implement workflows consistent with ECM requirements.
  • Ensure documentation standards are met to support accurate reporting and coordination of care.
  • Work with ECM providers, IT, and program staff to ensure accurate data capture and reporting processes.
  • Serve as a liaison between clinical / program teams and data / analytics teams.
  • Ensure data integrity and compliance with state and federal reporting requirements.
  • Establish repeatable processes for ongoing data quality monitoring and reporting.

The ideal candidate will possess :

  • Experience with reports required by state or federal governmental agencies.
  • Experience conducting data analysis.
  • Ability to conduct research and apply process improvement activities.
  • Ability to work under pressure with short deadlines.
  • Ability to work across divisions to provide support and coordinate work.
  • Excellent technical writing, editing, and proofreading skills.
  • Demonstrated attention to detail in processes and execution of work.
  • Proficient in computer systems and programs; specifically advanced level Excel, Power BI and other Microsoft Office Suite and Applications.
  • Experience with using statistical software e.g. R, SAS, Stata and Visio or similar platforms for data analysis and workflow creation.
  • Extensive verbal and written communication skills.
  • This is a limited term, at-will position. Limited Term Employees are assigned to assignments not to exceed three years. They receive medical and dental benefits, similar to regular employees, and a defined contribution retirement plan.
