Registered Nurse

Position Title : Acute Transitional Care Nurse (RN)

Department : Comprehensive Clinical Management

Work Location : Telecommuter – Must reside in Treasure Coast, Tampa, Citrus, Northeast Florida, or Orlando area

Work Schedule : Monday to Friday, 8 : 00 AM – 5 : 00 PM EST

Essential Functions :

• Prioritizes patient care needs and addresses emerging issues
• Meets with patients, families, and caregivers to discuss care and treatment
• Assists with patient follow-up across care settings
• Uses protocols and clinical resources to optimize outcomes
• Oversees discharge provisions and continuity of care
• Maintains communication with physicians, care teams, and families
• Understands cost consequences of care decisions
• Consults with physicians to implement care plans
• Coaches patients using motivational interviewing
• Coordinates with care team and ensures accountability
• Discusses DPoA and advanced directives
• Provides accurate benefit and resource information
• Documents patient information in electronic platforms
• Collaborates with providers under proper scope of practice
• Provides remote clinic coverage and travels as needed
• Performs additional duties as assigned
