Care Consultant

The Care Consultant plays a vital role on a dynamic team, delivering personalized care consultation and follow up support to individuals living with Alzheimer's disease or other dementias, as well as care partners and caregivers.

The primary responsibility of this position is to provide compassionate, expert guidance and support. This includes responding to dementia-related inquiries and situations with problem-solving assistance, decision-making support, crisis intervention, and referrals to local resources. The Care Consultant conducts clinical assessments to identify individual needs and develops tailored action plans. Services may include future planning support and in-depth education on dementia and caregiving strategies.

Care Consultations are provided in person, by phone, or via virtual platforms. This position serves individuals across the state of North Dakota and requires travel when virtual or phone-based consultations are not feasible.

This role is partially grant-funded and supports the fulfillment of a contract with the North Dakota Department of Health and Human Services, Aging Services Section.

This role also represents the Alzheimer's Association in various professional, business, and community settings, engaging in outreach efforts to advance the organization's mission and expand its impact.

Responsibilities :

• Utilize Solution Focused Brief Therapy (SFBT) techniques and skills while providing short- term telephone or in-person consultations in a professional and compassionate manner.
• Build rapport and engage constituents while utilizing clinical techniques to collaborate, assess, and enable constituents to pull from their expertise and knowledge of the situation to determine appropriate interventions and resources.
• Help constituents better understand Alzheimer's disease, the progression, and the needs of people living with the disease.
• Link constituents to education, support, local community resources, and emergency / crisis services if needed.
• Provide accurate and efficient documentation for call interactions including the presenting issue and goal, interventions, and action plan.
• Handle high emotions, trauma, and grief of callers with calmness and empathy to assess their current situation and take the next appropriate step(s).
• Follow established Association Helpline / Care Consultation policies, procedures, business rules, and clinical protocols.
• Uphold the Alzheimer's Association policy on client confidentiality.
• Integrate new knowledge and practices received through new hire and ongoing trainings.
• Achieves grant deliverables as assigned.
• Travels to communities in North Dakota to provide Care Consultation and services whenever virtual or phone consultations are unavailable.
• Conduct outreach activities to increase referrals and awareness of services.

Position Grade & Compensation : Grade - 105 Non-Exempt ( The Alzheimer's Association's good faith expectation for the salary range for this role is between $60,000-65,000)
