GQR Healthcare-Allied is seeking a travel Physical Therapist for a travel job in San Diego, California.

Experience :

2 years of PT HH experience (experience must be recent)

Home Health experience required

HCHB / OASIS Experience required (recent experience; no more than 1 year gap)  -  Must have HCHB / OASIS experience

Oasis E - after Jan 2023 experience is required

No more than 1 year gap

All documentation due same day

Homecare Homebase (Productivity points, minimum average26-28)

That could mean 3 people or 7 people

About 20-25 cases per week. 5-7points per day

Absolute must - true home health experience, Oasis / HCHB documentation

Home Health

Orientation : About 2-3 days of orientation. Modules, get intro to the team

Other Details :

Once a week they have to go into the office (Usually Tue, Wed and Thur). Mandatory in person

Hours : Branches are open 8am - 5pm, they may have patients who want to be seen before or after.

Homecare Homebase MUST be completed at the patient's home.

Absolute must - true home health experience, HCHB / Oasis documentation

Mileage will be reimbursed at .70 / per mile
