Join a dynamic healthcare team as an RN Care Manager, providing comprehensive coordinated care for adult patients across an interdisciplinary environment. You will develop, monitor, and evaluate individualized care plans while actively engaging in exceptional customer service and fostering respectful relationships with patients and colleagues.

Essential Functions

  • Conduct needs assessments and identify care options and services.
  • Communicate effectively with patients, families, and healthcare teams.
  • Manage patient data related to case management, care planning, and quality improvement analysis.
  • Monitor care delivery through patient rounds, documentation, and progress tracking.
  • Perform utilization reviews and assist nursing staff with patient care as needed.
  • Support rehabilitation training and education for staff when required.
  • Ensure compliance with healthcare regulations, including HIPAA privacy and security standards.
  • Perform additional duties as assigned by leadership.

Minimum Knowledge and Skills

  • Ability to develop and implement patient care plans.

  • Education and Experience

  • Completion of an accredited nursing program.
  • At least one year of experience as a registered nurse.
  • Preferred Qualifications

  • Bachelor of Science in Nursing (BSN).
  • Experience in care management.
  • Licensure

  • Current RN licensure in the applicable state.
