job_description.job_card.job_descriptionActive Compact LPN license. Minimum 2 years of clinical experience (care coordination preferred). Strong tech confidence—comfortable with EMRs, Microsoft Office, and digital tools. Excellent communication and problem-solving skills. Full-time, 40 hours / week | Monday–Friday between 8 AM – 6 PM (patient’s time zone). Orientation & Training (first 2 months) : $20 / hr. Monthly bonus potential : up to $525. Referral bonuses : up to $1,000. Timeline : The hiring process typically takes 2–3 weeks. Assessment : Qualified applicants complete a timed assessment covering computer literacy, basic math, and clinical knowledge. Interviews : Successful candidates will move forward to virtual interviews with our Recruiting Manager and Clinical Director.

What You’ll Do :
As a Patient Care Coordinator, you’ll play a vital role in our patients’ health journeys by :
- Conducting monthly telephonic outreach and assessing patient needs.
- Creating individualized care plans focused on physical, mental, and preventative health.
- Documenting visits in technology platforms and electronic health records (EHRs).
- Coaching patients through wellness, nutrition, and goal setting.
- Helping patients prepare for medical appointments and connecting them with resources.
