Requirements :

• Valid Kentucky Dental License (or willingness to obtain)
• Doctor of Dental Medicine (DMD) or Doctor of Dental Surgery (DDS) from an accredited institution
• Proficiency in comprehensive general dentistry ("bread and butter" procedures)
• Confidence in diagnosing and recommending appropriate treatment plans
• At least 1 year of professional experience as a practicing dentist
• Active DEA registration and controlled substance license for Kentucky
• Kentucky Medicaid provider number (or willingness to obtain)
