Telehealth Nurse Practitioner ON-SITE

Salary : $140k+ / yr based on experience

LaSante Health Center is seeking an ON-SITE Telehealth NP to deliver essential healthcare services to patients through telemedicine platforms. This role involves collaborating with healthcare teams, assessing patient requirements, diagnosing conditions, and devising treatment strategies.

Responsibilities :

• Plans and organizes patient care according to protocols and pathways.
• Collaborates with the healthcare team to develop and implement treatment plans.
• Conducts health assessments and orders necessary tests.
• Prescribes medications and medical equipment as per practice protocols.
• Evaluates treatment effectiveness and modifies plans as needed.
• Promotes professional growth and serves as a clinical resource.
• Assists Clinical Access Center staff when necessary.
• Completes additional tasks as directed by supervisor
