Summary
Advanced Practice Provider (NP or PA) assists and works collaboratively with the Palliative Care Medical Director, medical staff, and the Palliative Care Interdisciplinary Team (PCT) to provide patients with relief of stress and symptoms that accompany a serious, chronic, or life threatening illnesses, providing diagnosis, treatments and consultations.
Essential Duties and Responsibilities
- Works with a patient and his or her family to identify, control or alleviate physical and emotional discomfort and other symptoms that often accompany a serious health condition.
- Assesses and treats symptoms to promote maximum level of wellness and independence for patients with terminal illnesses.
- Works independently and covers after hours call for acute interventions as needed.
- Provides expert management of patients' symptoms, in addition to facilitating meetings with family members, and ensuring that delivery of care is coordinated across the health care setting.
- Provides resources to patients, caregivers and family members of available support groups and other services.
- Provides comprehensive information so patients can make well-informed, confident decisions about their care.
- Serves as an integral part and supports the goals of the Palliative Care Interdisciplinary Team which include :
- Managing pain control, alleviation of psychosocial distress, provide support for spiritual concerns and meet the practical needs of the patient throughout the continuum of health care.
- Improving a patient's and his or her family's access to information that can help them to understand the patient's condition and the range of treatment options available.
- Coordination of care across different settings by facilitating communication among providers, especially as changes occur in the patient's condition and needs.
- Formulates and prioritizes differential diagnoses of patients with serious illnesses, employing multifaceted treatment plans which include mind, body and spiritual interventions.
- Orders, interprets and evaluates diagnostic tests to identify and assess patient's healthcare needs.
- Monitors and manages the effects of cancer and cancer treatments as well as other serious diseases.
- Maintains precise and complete records of patients.
- Makes appropriate referrals.
- Reviews survivorship care plans.
- Provides care within the standardized procedures / protocols developed in collaboration with the collaborating / supervising physicians.
- Maintains positive, caring and supportive relationships with patients and care team.
- Communicates effectively with peers and physicians.
- Orchestrates daily care of patients in coordination with physicians and nursing staff.
- Demonstrates System Values in performance and behavior.
- Complies with System policies and procedures.
- Other duties as may be assigned.

Minimum Education / Experience
Physician Assistant
Certificate of completion from Completion of a Physician and 1-2 years relevant college or vocational school Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant.Nurse Practitioner
Graduation from a National League of Nursing (NLN), Commission on Collegiate Nursing Education (CCNE) or equivalent accredited school of nursing. The nurse shall have completed a nursing education program that included clinical experiences with actual patients under the supervision of qualified faculty.
