Join a dedicated home health care team serving patients throughout Washington County, MD. This role focuses on in-home therapy, OASIS assessments, and oversight of PTAs, providing a unique opportunity to impact patient recovery and independence in the comfort of their homes.

🧠 Responsibilities & Role Overview

Perform physical therapy evaluations and implement treatment plans in accordance with physician orders

Conduct OASIS assessments and complete documentation accurately and on time

Reassess patients and modify goals or interventions as appropriate

Provide in-home therapy visits to adult and geriatric populations

Oversee and support Physical Therapist Assistants within assigned territory

Recommend and assist with adaptive equipment as needed

Collaborate with medical providers, caregivers, and interdisciplinary teams to ensure coordinated care

Maintain compliance with HIPAA, state regulations, and agency policies

Participate in staff development and continuing education as required

✅ Requirements

Active Physical Therapy license in the state of Maryland

Recent home health experience strongly preferred

OASIS documentation experience required

HomeCare HomeBase (HCHB) EMR experience preferred

AHA BLS certification

Strong interpersonal, time management, and documentation skills

Must be comfortable working independently in a field-based role

Territory : All of Washington County, MD

No weekends, holidays, or on-call requirements

Field-based position – not remote

Oversight of PTAs is part of the role

Professional attire required (scrub color not specified)

Mileage reimbursement details available upon assignment
