Join Our Team as a Geriatric Medicine Physician!

Are you passionate about enhancing the lives of older adults? At The Southeast Permanente Medical Group , we are seeking a dedicated Geriatric Medicine Physician to provide specialized outpatient care for patients aged 65 and older. This is your chance to make a meaningful impact by focusing on assessments, treatment, and management of age-related conditions such as memory loss and falls.

Your Role

As part of our dynamic Elder Care team, you will collaborate with nurses, pharmacists, and social workers to optimize patient health and quality of life through both in-person and virtual consultations. Your responsibilities will include :

• Conducting telephone, virtual, and office visits.
• Assessing, treating, and recommending care for patients referred to Geriatrics.
• Documenting goals of care discussions and advance directives.
• Developing comprehensive care plans in collaboration with the patient’s Primary Care Physician (PCP).
• Responding to MD advice and referrals in a timely manner.
• Collaborating with specialists to provide comprehensive care.
• Supervising Geriatric Nurse Practitioners and providing feedback on care plans.
• Educating primary care and other specialists about geriatric patient care.
