JOB DEFINITION :

Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.

EDUCATIONAL REQUIREMENTS :

A Bachelor's degree in social work, psychology, rehabilitation counseling, or related field. Licensure with the NYS Education Department in a specific field is preferred.

ADDITIONAL REQUIREMENTS :

• Current New York State Driver's License
• Clean DMV Record
• Proof of Motor Vehicle Insurance

EXPERIENCE REQUIRED :

Two years' experience working with the chronically mentally ill (Masters level) recommended. Three years' experience working with the chronically mentally ill (Bachelor's level) recommended. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and / or experience in integrated mental health and substance abuse assessment and treatment.
