Available  schedule :   Work schedule Monday-Friday 8 : 30am - 5pm with 1 weekend every 6   weeks

What You’ll Do Provide skilled nursing care and assessments in members’ homes

Develop, update, and carry out patient care plans

Administer medications and perform nursing procedures

Educate and support patients, families, and caregivers

Coordinate care with physicians and other healthcare professionals

What We’re Looking For

• Current NY Registered Nurse (RN) license
• 1+ year of recent nursing experience (home health)
• Experience with elderly or frail populations (a plus!)
• Reliable car, valid driver’s license, and insurance
