Family Nurse Practitioner – Southwest Arizona

Job Responsibilities
As a Family Nurse Practitioner in this role, you will :
- Provide comprehensive primary care services including preventative care, acute and chronic disease management, and patient education
- Evaluate, diagnose, and manage patients independently in a clinic setting
- Utilize an integrated, team-based care model in collaboration with physicians, nurses, case managers, and behavioral health staff
- Order and interpret diagnostic tests, prescribe medications, and coordinate specialty referrals when needed
- Document all patient care accurately and in a timely manner using an EHR system
- Participate in community health outreach, educational initiatives, and team-based meetings
- Support continuity of care through effective patient communication and clinical follow-ups
- Maintain compliance with Arizona licensing standards and organizational protocols
- Rotate in emergency call schedules as assigned (evenings / weekends as needed)
- Participate in quality assurance, professional development, and improvement initiatives
Minimum Qualifications
Graduate of an accredited Nurse Practitioner programActive Arizona Registered Nurse and Advanced Practice licensesNational board certification as a Family Nurse PractitionerValid DEA license and BLS / CPR certificationDemonstrated cultural competence and commitment to inclusive careStrong interpersonal, documentation, and clinical decision-making skillsLevel One Fingerprint Clearance Card (or ability to obtain)Valid Arizona driver’s licenseNew graduates welcome to applyPreferred Qualifications
Two (2) years of experience in a community health or underserved settingBilingual in English and SpanishExperience with Electronic Health Records (EHR)Compensation & Benefits
