MINIMUM MANDATORY qualifications

POSITION PURPOSE

This position is responsible for Care Coordination for perinatal clients and children that are patients of our medical clinics and hospital. The purpose of Care Coordination is to reduce maternal and child health disparities by identifying and navigating access to a wide range of social and medical resources available. Care Coordinators work closely with clients, social service agencies, medical practitioners, and behavioral health providers to develop and coordinate a Care Plan that is culturally / linguistically appropriate, trauma-informed, strength-based, and in full compliance with the Care Coordintion model. Care Coordinator uses approved on-line data systems and paper charting to provide required documentation and data for all Care Coordination clients. The Care Coordinator works with billing departments, Care Coordination supervisior and auditors to implement the Care Coordination model and required documentation to full fidelity, and to ensure that Care Coordination is contracted for and billed to insurance / Medicaid.

Experience :

• Experience in social services, healthcare, or related field
