Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Join Sunbelt Staffing as a Home Health RN in Hammond, LA! This full-time, 13-week contract offers the opportunity to deliver meaningful, patient-centered care in a supportive home health environment. The ideal candidate will have prior agency home health experience and be proficient in OASIS documentation and Homecare Homebase (HCHB), bringing both skill and compassion to every patient interaction.

Contract Details :

  • Location : Hammond, LA, and surrounding areas
  • Company : Sunbelt Staffing, partnered with a Nationwide Home Health Agency
  • Position Type : Full-Time Contract, Home Health Field RN
  • Contract Length : 13 weeks, with possibility of extension
  • Hours : Full-Time, Scheduled 40 hours per week - 32 hours guaranteed
  • Schedule : Monday-Friday, 8 AM - 5 PM, with occasional on-call (Typically, one weekend per month)
  • Productivity : 25per week, 30-35 visits per week
  • Compensation : Negotiated directly with client based on experience and territory budget; paid hourly - not by visit; blended pay packages available for travelers; weekly pay
  • Mileage : .48c visit to visit, 150-250 miles per week expected
  • Benefits : Medical, dental, vision, 401k

Key Responsibilities :

  • Conduct comprehensive patient assessments using the Oasis documentation system.
  • Develop and implement individualized care plans based on patient needs.
  • Utilize Homecare Homebase for accurate and timely documentation.
  • Provide skilled nursing care and education to patients and their families in their homes.
  • Coordinate with interdisciplinary team members to ensure continuity of care.
  • Manage patient caseload and ensure timely visits.
  • Participate in on-call rotation, including one weekend per month.

    • Qualifications :

  • Active LouisianaRegistered Nurse (RN) License
  • Active American Heart Association or American Red Cross BLS Certification required
  • At least 1 year of agency home health experience required
  • Proficiency in Oasis and Homecare Homebase required
  • Reliable transportation required
