Description :

Provide primary care for all ages.

Offer wellness and prevention care.

Treat minor injuries and common illnesses.

Manage chronic conditions, like diabetes and high blood pressure.

Provide behavioral care.

The clinic serves a diverse patient population including pregnant individuals, infants, children, adults, and the elderly.

Patients seen are typically moderately stable to moderately unstable, with or without complex acute or chronic illnesses and social determinants of health barriers that require medical and nursing interventions.

This role focuses primarily on telephone triage and advice.

Respond to patients' urgent needs both in-clinic and via telephone.

Flexibility and a nimble attitude are essential.

Visit types offered include in-person, virtual, and telephone.

Services include telephone and in-clinic triage, behavioral health and counseling services, reproductive health services, transgender health care, prenatal and post-partum care, pediatric care, medication-assisted treatment, and health maintenance with preventive care.

Experience in charting systems is preferred.

Epic experience is required for submission.
