Requirements :

Board Certified Neurological Surgery - Required or Truly BE (0-5 years of residency completion)

Active CO License - Required or IMLC (LOQ in hand at time of submission)

Active ACLS BLS - Required for credentialing

Active DEA - Required for credentialing (site will consider a transferred DEA however must keep this DEA for the duration of the assignment not just for shifts worked.)

Must disclose # of shifts per month provider can offer to the team ongoing - Required

Candidates must be available for any dates detailed in the job description to be considered for ongoing dates - Required

Must acknowledge candidate understands the EMR training requirements set forth in job description and will complete in this order to avoid missing their start date - Required

Clean Malpractice / Background (December shifts only) - Required
