- Position Summary :
As a
- Home Health Occupational Therapist
- , you will provide therapy services to patients in their homes, focusing on helping them regain independence and improve their quality of life. You'll develop personalized care plans, set functional goals, and work closely with patients, families, and other healthcare professionals.
- Key Responsibilities :
- Conduct in-home assessments to evaluate patients physical and cognitive abilities
- Develop individualized treatment plans based on patient needs and goals
- Deliver therapeutic interventions to improve ADLs (activities of daily living)
- Educate patients and caregivers on home safety, adaptive equipment, and self-care techniques
- Collaborate with nurses, PTs, and other clinicians as part of the interdisciplinary care team
- Accurately document visits and progress in EMR system
- Ensure compliance with all regulatory and agency standards
- Qualifications :
- Licensed Occupational Therapist (OT) in the state of Ohio
- Graduate of an accredited OT program
- Minimum of 1 year of experience in home health (preferred, but not required)
- Valid drivers license and reliable transportation
- Strong communication, organizational, and time management skills
- Comfortable working independently in the field
