As a RN Case Manager You will :
Assess / monitor physical, emotional, and psychological needs of patientsCreate hospice care plans that align with the patients wishes and goalsDirect nursing care : administering medications, treatments, and interventionsProvide pain and symptom managementEducate and support the patient’s family and caregiversCollaborate with interdisciplinary teamMaintain accurate and timely documentationParticipate in on-call rotation as required by local branchQualifications
Graduate of an accredited nursing school with current licensure to practice in state of operationOne year nursing experience required, 3+ years preferredExperience in hospice or a similar setting preferredKnowledge of the hospice philosophy of careCommitment to clinical and documentation excellenceMust be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order
