Registered Nurse (RN) - Hospice
Protouch Staffing is seeking a compassionate and skilled Registered Nurse (RN) with experience in hospice care to join our growing home health team. In this role, you will provide specialized care to terminally ill patients in the comfort and privacy of their own homes, helping them manage their physical and emotional needs during end-of-life care. This is a unique opportunity to provide personalized, one-on-one care and build meaningful relationships with patients and their families.
Responsibilities
- Conduct comprehensive in-home hospice assessments to determine the patient's needs and eligibility for services.
- Develop, implement, and evaluate individualized, patient-centered nursing care plans in collaboration with the patient, family, physician, and interdisciplinary team.
- Provide skilled nursing care to terminally ill patients, including pain and symptom management , medication administration, and wound care.
- Educate and support patients and their families on the hospice philosophy, end-of-life care, and the dying process.
- Administer and monitor the effects of prescribed medications, providing patient and caregiver education on proper administration, side effects, and adherence.
- Observe and document changes in the patient's physical and mental condition, including mood, behavior, appetite, and cognitive function.
- Coordinate care with other healthcare professionals to ensure continuity of care and optimal patient outcomes.
- Maintain accurate, timely, and detailed clinical documentation using an electronic health record (EHR) system.
- Act as a patient advocate, ensuring their health and safety needs are met.
- Provide bereavement support and resources to families.
Qualifications
Current and unrestricted Registered Nurse (RN) license in the state of MinnesotaA minimum of [e.g., one to two] years of recent clinical experience as a Registered Nurse.Hospice experience is required.Strong critical thinking, analytical, and problem-solving skills.Excellent communication and interpersonal skills, with the ability to build rapport with diverse patient populations.Ability to work independently and manage time effectively.Proficiency with electronic health records (EHR) and basic computer skills.Valid driver's license and reliable transportation.Compensation Details
