Physical Therapist Assistant - Home Health

Join our team as a dedicated Physical Therapist Assistant in Home Health, where you will work under the supervision of physical therapists to help patients regain movement and manage pain in their own homes.

Key Responsibilities :

• Assist physical therapists in providing physical therapy treatments and procedures as outlined in the care plan.
• Observe and document patients’ status before, during, and after therapy, reporting progress and concerns to the supervising physical therapist.
• Help patients perform specific exercises and use therapeutic equipment safely in the home environment.
• Educate patients and family members about exercises and activities to continue between visits.
• Adapt interventions as needed within the scope of the treatment plan and state laws.
• Ensure all documentation is accurate and completed in a timely manner according to agency and regulatory standards.
