Hawaii Licensed Disease Management Nurse

A company is looking for a Registered Nurse Disease Manager, remote with security clearance.

Key Responsibilities

Perform assessments and coordinate care for dependents of Active Duty and eligible retirees

Provide education and training related to disease management

Work remotely while adhering to Hawaii residency and timezone requirements

Required Qualifications, Training, and Education

Bachelor's Nursing Degree

Hawaii state nursing license, with preference for additional compact state licensure

At least 2 years of experience as a Registered Nurse Disease Manager, preferably in a remote setting

Preferred certifications in disease management, such as Chronic Care Professional or Certified Diabetes Educator
